Sign Templates not saving all elements.

Hi, 

I am trying to save a template consent form in Box, to send to many individual recipients. I created a template document with a standard message, and multiple sign elements: check boxes, a signature, a full name, a date. I then save this as a template. The template successfully saves my email message and my document, but NOT my elements (i.e., the checkboxes and signature fields are all gone and I need to manually specify them again).

I need to specify these before forms are sent out; I am working with populations that may not be able to place these appropriately themselves. Can anyone help?

Hi, I am having the same issue. I created a form template with various input fields but when I try to use the template none of the input fields saved in the template are showing.

Same issue, seems broken, why have this feature if it does not work.

For those who came here looking for answers: Try sending the document for signature FIRST, then creating the template from the SENT file. This solved the issue for me. 

Not sure why Box can't just add a solution to these threads.
