I recently purchased a Box Business account and need assistance creating an embedded widget to route document uploads to separate folders within Box. For example, I would like the option to upload documents to folder1 or folder2 by adding access to my business website. However, I don't want 2 widgets on the site. How can I create a drop-down option (folder1 or folder2) on the site instead of 2 separate embedded widgets?

I would also like the option of the person uploading the documents to add their first and last name along with their email address. How do I do this?
