I have updated a file (Excel) on my iPad. The update shows on the iPad and on my MAC. It shows as having been updated on my iPhone, but an old version of the file is there.

Have you already updated your Box for iPhone app? If not can you update the app on your iPhone and see if this helps.
If the issue persists, try to logout and then re-login to your iPhone app and check if the same file updated to the latest version.
