Confirmation Email Fails to send.

Quite often after adding a new user when they try to access box for the first time the confirmation email fails to send.  I advise them to click the resend confirmation email and it will send.  What is causing the initial confirmation email to fail?  Is this a Known issue?  Is there a solution to prevent this issue?
