2FA not working

I have set up 2FA for my new Box Account.  My first attempt to log in worked perfectly using an Authenticator App.  My subsequent log in attempts I have NOT been required to provide a 2nd verification.  My account settings look like I'm set up to require 2FA on every log in attempt.  Am I missing something?
